Applying for a Grant
Are you part of a community organization that is developing or expanding programs and services for individuals with intellectual/developmental disabilities (IDD)? If so, you may be able to apply for grant funds from Foothills Gateway to support your program.
Foothills Gateway, Inc. is now accepting grant applications for the 2026-27 Fiscal Year (7/1/2026-6/30/2027).
Step 1: Do You Qualify?
To see if you qualify to submit an application, first answer these questions:
- Do your clients/participants receive case management services from Foothills Gateway and reside in Larimer County, Colorado?
- Does your proposed program provide services for individuals with intellectual or developmental disabilities (IDD)?
- Is your organization a PASA (Program Approved Service Agency) or a Medicaid Provider Agency?
- Does your proposal target a specific gap in local services to the IDD population?
- Does your organization have experience serving the target population?
- Do you have a plan for sustaining the proposed program financially after the grant period is over?
If you answered “YES” to all of these questions, you qualify to submit a grant application.
Step 2: Choose Your Funding Category
Foothills Gateway, Inc. is now accepting grant applications for the 2026-27 Fiscal Year (7/1/2026-6/30/2027) for funding in the following categories:
- Social, Vocational/Employment, and Recreational Opportunities – Improving access to community integration and recreational opportunities.
- Behavioral and Mental Health Services – Providing behavioral health services beyond waiver or Medicaid State Plan–defined services.
- Education and Independence – Providing educational opportunities with the goal of increasing autonomy.
- Medical and Dental Needs – Options for medical and/or dental services that are not currently available under the Medicaid State Plan or through a Medicaid Waiver program.
- Training and Support for Families and Others – Training and support for caregivers, family members, and the general public on issues related to people with IDD.
- System Gaps – Projects or services that bridge gaps or unmet needs within the IDD system, as well as between the IDD system and other systems. Services funded through this category would be above and beyond what is available through Medicaid/Medicaid waiver program or state funds.
- Capital Projects – Improvement or creation of spaces in Larimer County which would primarily be used to provide services to individuals with IDD. Capital projects may only be considered when the requestor is the property owner.

Frequently Asked Questions
How Much Money Can Be Requested?
The maximum amount that can be allocated for an approved project is $50,000 per fiscal year. (For capital projects, applicants may submit grant requests exceeding $50,000.) Every request is considered on a case-by-case basis and awards are made if funding allows.
All awarded grant funds should be spent within the allocated July-June fiscal year when possible. Exceptions must be requested in writing. If grant funds are unable to be spent within the fiscal year allocated, the grantee must return to the Funder any part of the grant amount unencumbered at the end of the grant period and identified in the final financial report.
Who Can Apply for a Foothills Gateway Grant?
Nonprofit community organizations and provider agencies (PASAs) operating in and serving individuals in Larimer County, Colorado, may apply. Funding must be used solely for the benefit of individuals who have been determined eligible for IDD services and who receive case management services through Foothills Gateway, Inc. and reside in Larimer County. Preference will be given to PASAs (Program Approved Service Agencies) and Medicaid provider agencies.
When applying, organizations must show how their proposed project will benefit individuals with IDD. Individuals served through granted dollars must have developmental delays or intellectual/developmental disabilities as outlined in 10 CCR 2505-10 8.7000 and 8.6000, in addition to meeting Larimer County residency and FGI case management and/or service provision requirements. Grant applications must provide supporting documentation such as a line-item budget, quotes/bids for service, proof of ownership for capital expenses, etc.
Grant funds will not be awarded for legal fees, real estate/rent, travel or vacations, serving non-IDD populations, or for supplementing Medicaid payments. The funds cannot be used if the intended service is available through a Medicaid Waiver, Medicaid State Plan, or State General Funds.
What Are the Reporting Requirements?
Funded organizations must provide quarterly and year-end reports on their project’s progress according to the following requirements:

- Describe the progress made toward the goals and objectives provided in the grant application.
- Describe the impact this grant had on the population you serve.
Lessons Learned
- What worked and did not work in your proposed plan?
- What, if anything, would you do differently next time?
Sustainability
- Will you continue this program? If so, how?
- If you do not plan to continue this work, please explain why.
- What suggestions do you have to make more long-term change?
Financials
- All grant recipients must submit a completed financial report showing the use of funds in alignment with the grant application and agreement. The report must include income and expenditure details compared to the approved budget from the grant application. Explain any variances.
