Early Intervention

Early Intervention Colorado and Foothills Gateway, Inc. coordinate services for children that qualify for supports due to an intellectual/developmental disability (IDD), an automatically qualifying condition, or if the child has a parent with IDD.  Early Intervention is a coaching-based program that provides supports to caregivers to help their child learn and grow.  This program connects families with early intervention services such as: occupational therapy, speech language pathology, physical therapy, and other services to encourage and enhance development. This program serves children 0-3 years old and their families.

 

To make a referral for Larimer County’s Early Intervention program use one of the three options below:

> Call 833-REFER-EI (833-733-3734)
> Complete the online referral form OR download and fax the printable referral form. Fax to 303-866-5944
