  • EARLY INTERVENTION SERVICE COORDINATOR

    The Early Intervention Service Coordinator is responsible for advocating, planning, coordinating, and monitoring services and supports provided to families of infants and toddlers eligible for Part C Early Intervention services from an assigned caseload of children. The Service Coordinator will facilitate initial and ongoing Individualized Family Service Plan (IFSP) meetings, complete family assessment, support the family to ensure services are being delivered in accordance with the IFSP, monitor the provision of services, supports, and recommendations identified in the IFSP, assist in obtaining identified services and supports, maintain a master record and all necessary documentation, serve as the primary contact for obtaining and disseminating information, and facilitate and mediate conflict resolution. Inherent in all these functions shall be activities supporting families Rights and Procedural Safeguards under Part C, support, empowerment, and advocacy. Must have a bachelor’s level degree of education; or Five years of experience in the field of developmental disabilities; or some combination of education and experience appropriate to the requirements of the position. Driving is a requirement for this position.

    -$200 hiring bonus after 90 days of employment.

    -1 full-time position at 40 hrs. /wk., $23.14-$25.45/ hr. DOE/DOQ

    -Remote/Mobile position–must be able to attend trainings and meetings in Fort Collins, CO & throughout Larimer County

  • DRIVER/VEHICLE MAINTENANCE TECH ASSISTANT

    Position Closes: 06/21/2024

    This position works with people with intellectual/developmental disabilities. Responsibilities include but are not limited to assisting the Vehicle Maintenance Technician with monthly inspections of agency vehicles, paperwork documentation and driving passengers to and from places of employment, their residence, and other locations. Driving various agency vehicles is a requirement for this position and all duties are performed within the Foothills Gateway service area. Monday-Friday, hours vary depending on the specific transportation needs for the day but typically will occur between 7am-4:30pm.

     

    -$200 hiring bonus after 90 days of employment.

    -1 part-time position up to 29.75 hrs. /wk. starting at $15.76-$17.34/hr. DOE/DOQ

    -5 paid holidays, accrual of paid time off based on hours worked, paid sick leave, paid EAP benefit and/or other benefits.

  • DIRECT SUPPORT PROVIDER – ADULT CARE SERVICES

    The Adult Care Services program provides temporary respite care for adults with Intellectual and Developmental disabilities at our respite house in Fort Collins. Direct Support Providers have the rewarding opportunity to work with and support adults with activities of daily living, social and recreational activities. Must be able to attend trainings and meetings at the main facility. Shifts include weekdays, evenings, weekends, and overnights. Some intermittent schedule availabilities may be required. This is a great opportunity to provide excellent and meaningful care and supports to individuals with varying abilities and care needs & help make their time away from home a wonderful experience. Driving is a requirement for this position.

     

    -$200 hiring bonus after 90 days of employment.

    -Part-time positions available up to 29.75 hrs. /wk., $16.29-$17.92/hr. DOE/DOQ, current CO DSP minimum wage for overnights

    -5 paid holidays, accrual of paid time off based on hours worked, paid sick leave, paid EAP benefit and/or other benefits.
