Apply NowThe Lead Comprehensive Case Manager will provide case management services in a person-centered manner and integrate person-centered practices into his or her supervision of case managers. In addition to maintaining a caseload and all of the responsibilities associated with the caseload. The Lead Comprehensive Case Manager also supervises Comprehensive Case Managers. Spanish bilingual preferred with a $1.00 increase in pay rate.
Requirements:
- Bachelor’s degree required; or five years of relevant experience in the field of LTSS, which includes Developmental Disabilities; or some combination of education and relevant experience appropriate to the requirements of the position.
- Knowledge of cognitive disabilities: cause, treatment, services for support, education, employment, recreation, therapy, and related medical terms/medications.
- Driving is a requirement for this position. Must be able to drive company vehicles.

IDD CASE MANAGER – SLS/CES

Apply NowThe Case Manager is responsible for a variety of duties, which could include responding to contacts seeking information regarding services and supports for individuals with intellectual and developmental disabilities, assembling information about the individual for eligibility determination, providing Case Management, being responsible for the assessment planning, coordination, and monitoring of services and supports provided to individuals with intellectual/developmental disabilities and their families as appropriate. Spanish bilingual preferred with a $1.00 increase in pay rate.
Requirements:
- Bachelor’s degree required; or five years of relevant experience in the field of Long-Term Services & Supports, which includes Developmental Disabilities.
- Driving is a requirement for this position.
- Prior case management and/or advocacy experience strongly preferred.
- Must be comfortable working with and near individuals with intellectual/developmental disabilities.
- Must display a high degree of responsibility, initiative, and interpersonal and communicative skills.

Direct Support Provider – Adult Care Services – Part Time

Apply NowThe Adult Care Services program provides temporary respite care for adults with Intellectual and Developmental
disabilities at our respite house in Fort Collins. Direct Support Providers have the rewarding opportunity to work
with and support adults with activities of daily living, social and recreational activities. Must be able to attend
training courses and meetings at the main facility. Required availability for shifts include evenings, weekends,
and overnights. Some intermittent schedule availabilities may be required. This is a great opportunity to provide
excellent and meaningful care and supports to individuals with varying abilities and care needs and help make
their time away from home a wonderful experience. Driving is a requirement for this position.Duties/Responsibilities:
- Ensure positive, safe environment for individuals receiving services.
- Complete daily household chores to ensure a clean and safe living environment.
- Monitor the health and safety of individuals.
- Assist individuals with medication as need indicates.
- Assist, support, promote, and train individuals with independent life skills and a person-centered approach.
- Assist individuals with personal care/hygiene when needed.
- Manage crisis intervention as necessary (medical emergencies, behavioral situations, incident reports, etc.).
- Report all mistreatment, abuse, neglect, exploitation, other incidents and concerns to Program Director.
- Driving is a required function of this job and may include driving for community activities and other appointments.
-1 Part-Time position available up to 29.75 hrs/wk., $18.75-$20.63/hr. DOE/DOQ, sleep rate for overnight shift is $17.00/hr.
-5 paid holidays, accrual of paid time off based on hours worked, paid sick leave, paid EAP benefit, 401K and Colorado Paid Family and Medical Leave Insurance Program (FAMLI).
