  • Case Management Case Aide – LTSS

    Position Closes: 07/21/2026

    The LTSS Case Management Case Aide is responsible for a variety of duties, which include tracking and coordination of restricted funds requests, supporting case management functions, and maintaining the LTSS provider list and provider portal access. The LTSS Case Management Case Aide facilitates contact with FGI LTSS Case Managers, vendors, and provider agencies.  Supports with case management tasks, including monthly or monitoring contacts, on an as needed basis during times of high volume. Coordinates scheduling of Person-Centered Thinking training, as needed; support with monitoring and distribution of exception requests to case managers; support with data gathering and administration support for LTSS case managers, leads, and Associate Directors. This position works on-site in the Foothills Gateway building, in community settings, and has remote work options. Spanish bilingual preferred with a $1.00 increase in pay rate.

    Requirements:
    • Bachelor’s or Associate degree is helpful but not required. Some combination of education and *relevant experience appropriate to the requirements of the position.
    • Case management and/or advocacy are strongly preferred.
    • Knowledge of cognitive disabilities: cause, treatment, services for support, education, employment, recreation, therapy, and related medical terms/medications.
    • Driving is a requirement for this position. Must be able to drive company vehicles.

  • Case Management Case Aide – Intake

    The Intake Case Management Case Aide will be responsible for a variety of duties, which include responding to initial calls to Foothills Gateway regarding services and supports, assessing and screening calls to find the best options for the member, completing the Colorado Intake Screen Tool for new referrals. Facilitating contact with FGI Intake Case Managers or ongoing Case Managers. Completion of Long-Term Care Assessments in times of high volume.   • Direct Support Provider – Adult Care Services – Part Time

    Position Closes: 07/21/2026

    The Adult Care Services program provides temporary respite care for adults with Intellectual and Developmental
    disabilities at our respite house in Fort Collins. Direct Support Providers have the rewarding opportunity to work
    with and support adults with activities of daily living, social and recreational activities. Must be able to attend
    training courses and meetings at the main facility. Required availability for shifts include evenings, weekends,
    and overnights. Some intermittent schedule availabilities may be required. This is a great opportunity to provide
    excellent and meaningful care and supports to individuals with varying abilities and care needs and help make
    their time away from home a wonderful experience. Driving is a requirement for this position.

    Duties/Responsibilities:
    • Ensure positive, safe environment for individuals receiving services.
    • Complete daily household chores to ensure a clean and safe living environment.
    • Monitor the health and safety of individuals.
    • Assist individuals with medication as need indicates.
    • Assist, support, promote, and train individuals with independent life skills and a person-centered approach.
    • Assist individuals with personal care/hygiene when needed.
    • Manage crisis intervention as necessary (medical emergencies, behavioral situations, incident reports, etc.).
    • Report all mistreatment, abuse, neglect, exploitation, other incidents and concerns to Program Director.
    • Driving is a required function of this job and may include driving for community activities and other appointments.

    -2 Part-Time position available up to 29.75 hrs/wk., $18.75-$20.63/hr. DOE/DOQ, sleep rate for overnight shift is $17.00/hr.

    About the Role

    Reporting to the Board of Directors, the CEO is responsible for the overall leadership, strategy, and performance of the organization. You will oversee all programs, operations, and partnerships while ensuring alignment with FGI’s mission, vision, values, and regulatory obligations.

    FGI operates as:

    • A Case Management Agency (CMA) under Colorado Health Care Policy and Financing (HCPF).
    • The designated Community Centered Board (CCB) for Larimer County.
    • The Early Intervention Broker (EIB) for the Colorado Department of Early Childhood (CDEC).

    This role requires deep expertise in compliance, risk management, and regulatory systems.

    • Build and lead a high-performing executive team.

    • Serve as FGI’s primary representative with state agencies (HCPF, CDHS, CDPHE) and community partners.

    • Build strong partnerships across government, service providers, and the community.

    Who You Are

    Required Qualifications

    • Bachelor’s degree and a minimum of 8+ years of senior executive leadership in human services, healthcare, behavioral health, or a related nonprofit environment.
    • Demonstrated success managing complex, multi-funded programs and budgets.
    • Deep experience working with a Board of Directors and nonprofit governance structures.
    • Strong knowledge of regulatory systems and compliance practices.

    Preferred

    • Master’s degree in public administration, nonprofit management, business, or related field.
    • Experience with Colorado’s CMA/CCB systems and applicable regulations.
