• Early Intervention Service Coordinator

  Position Closes: 10/16/2026

  The Early Intervention Service Coordinator will be responsible for the planning, coordinating, and monitoring services and supports provided to infants and toddlers eligible for Federally funded Part C services and State funded Early Intervention services. Also, the EI Service Coordinator will have an assigned caseload of individuals for whom they will be responsible to advocate. Additionally, the EI Service Coordinator will facilitate initial and ongoing Individualized Family Service Plan (IFSP) meetings, complete family assessments, and support the family to ensure services are in accordance with the IFSP.

  Furthermore, they will monitor the provision of services, support, and recommendations identified in the IFSP. As well as assisting in obtaining identified services and support. Also, they will maintain a master record and all necessary documentation, serve as the primary contact for obtaining and disseminating information. In addition, they will facilitate and mediate conflict resolution. Inherent in all these functions shall be activities supporting families Rights and Procedural Safeguards under Part C, support, empowerment, and advocacy. Bilingual in Spanish is preferred.

  Requirements/Responsibilities:

  • Must have a bachelor’s level degree of education; or, Five years of experience in the field of developmental disabilities; or some combination of education and experience appropriate to the requirements of the position.
  • Experience in the field of cognitive disabilities resource coordination. Working with children with cognitive disabilities and their families, and/or advocacy strongly preferred.
  • Driving is a requirement.

  – 1 full-time positions available at $25.43 – $27.97/hr. DOE/DOQ.

  -Remote/Mobile position – must be able to attend in-person training and meetings in Fort Collins, CO and throughout Larimer County.

• Direct Support Provider – Adult Care Services – Part Time

  Position Closes: 10/23/2026

  The Adult Care Services program provides temporary respite care for adults with Intellectual and Developmental
  disabilities at our respite house in Fort Collins. Direct Support Providers have the rewarding opportunity to work
  with and support adults with activities of daily living, social and recreational activities. Must be able to attend
  training courses and meetings at the main facility. Required availability for shifts include evenings, weekends,
  and overnights. Some intermittent schedule availabilities may be required. This is a great opportunity to provide
  excellent and meaningful care and supports to individuals with varying abilities and care needs and help make
  their time away from home a wonderful experience. Driving is a requirement for this position.

  Duties/Responsibilities:

  • Ensure positive, safe environment for individuals receiving services.
  • Complete daily household chores to ensure a clean and safe living environment.
  • Monitor the health and safety of individuals.
  • Assist individuals with medication as need indicates.
  • Assist, support, promote, and train individuals with independent life skills and a person-centered approach.
  • Assist individuals with personal care/hygiene when needed.
  • Manage crisis intervention as necessary (medical emergencies, behavioral situations, incident reports, etc.).
  • Report all mistreatment, abuse, neglect, exploitation, other incidents and concerns to Program Director.
  • Driving is a required function of this job and may include driving for community activities and other appointments.

  -1 Part-Time position available up to 29.75 hrs/wk., $18.75-$20.63/hr. DOE/DOQ, sleep rate for overnight shift is $17.00/hr.

  -5 paid holidays, accrual of paid time off based on hours worked, paid sick leave, paid EAP benefit, 401K and Colorado Paid Family and Medical Leave Insurance Program (FAMLI).
