SLS Waiver

Supported Living Services Waiver (SLS)

Ages 18 and older

Allows adults (18 and older) with an intellectual or developmental disability (I/DD) who live independently or with family to receive supported living services in the person’s home or community. 

 

 

Qualifying for SLS Medicaid Waiver services

  • Must be 18 years old or older
  • Live independently or in the family home
  • Have been determined to have a developmental disability before the age of 22
  • Have a disability as defined by the Social Security Administration (SSA)
  • Meet financial determination requirements for long-term care Medicaid as determined by your local Department of Human Services (DHS)
