What Is Community First Choice?
Community First Choice helps people live safely and independently at home instead of a nursing facility. CFC is a Medicaid program that provides expanded services and options for Home and Community-Based Services (HCBS) to Health First Colorado members who need long-term care. Individuals can choose who provides their care (either a service agency or someone they trust).
Services Available Through CFC
- Personal Care (daily tasks you are unable to perform yourself, such as hygiene, eating, or exercise)
- Homemaker (such as light housekeeping, meal prep, and shopping)
- Health Maintenance Activities
- Personal Emergency Response Systems (PERS)
- Medication Reminders
- Transition Setup (from an institutional setting to a home or community-based setting)
- Home Delivered Meals
- Remote Supports Technology
