    Skilled Nursing Facility Referrals & Admissions (SNF)

    All Ages

    Allows individuals of all ages to receive 24-hour skilled nursing or medical care, rehabilitation, or health related services in an institutional setting.  

     

    Note: As the Case Management Agency (CMA), Foothills Gateway, Inc. provides assessments and referrals to Skilled Nursing Facilities, but does not provide ongoing case management or oversight.

    • Must be 65 years old or older and/or meet other criteria
    • Meet the functional level of care requirements which is determined through an assessment process
    • Meet financial determination requirements as outlined in the Colorado Code of Regulations and as determined by your local Department of Human Services (DHS)
