PACE

Programs of All-Inclusive Care for the Elderly (PACE)

Ages 55 and older

Allows adults (55 and older) to receive comprehensive health care services while continuing to live in the community as long as is medically and socially feasible. 

 

Note: As the Case Management Agency (CMA), Foothills Gateway, Inc. provides assessments and referrals to PACE, but does not provide ongoing case management or oversight for these programs.

  • Must be 55 years old or older
  • Live in the service area of the PACE organization
  • Meet nursing facility level of care requirements
  • At the time of enrollment, be able to live in a community setting without risk to health or safety

 

Available PACE Services

The PACE benefit package for all participants, regardless of the source of payment, must include the following:

  • All Medicare-covered services
  • All Medicaid-covered services as specified in the Medicaid State Plan (Health First Colorado)
  • Other services determined necessary by the interdisciplinary team to improve and maintain the participant’s overall health status
