SLS-CES Case Manager

The Case Manager will be responsible for a variety of duties, which could include responding to contacts seeking information regarding services and supports for individuals with cognitive disabilities, assembling information about the individual for eligibility determination, providing Case Management services until the person can be admitted into services, being responsible for the assessment planning, coordination, and monitoring of services and supports provided to individuals with intellectual/developmental disabilities and their families as appropriate.
Requirements:
- Bachelor’s degree required or five years of relevant experience in the field of LTSS.
- Five years’ experience in Case Management and/or advocacy strongly preferred.
- Driving is a requirement. Must have a valid driver’s license with an acceptable driving record.

– 1 full time position available at $25.35-$27.89/hr. DOE/DOQ.
– Approximate accrual of 15 days per year paid time off to start, paid holidays, paid sick leave, and a full range of Medical, Dental, Vision, Life and Disability and EAP, in addition, we offer 401K and Colorado Paid Family and Medical Leave Insurance Program (FAMLI).
