LEAD LTSS CASE MANAGER

The Lead LTSS Case Manager is responsible for a variety of duties, which include responding to contacts seeking information regarding services and supports for members with qualifying disabilities, providing Case Management services, by planning, coordinating, and monitoring of services and supports provided to members who are elderly, blind, and members with disabilities or significant mental illness. The Lead LTSS Case Manager will provide case management services in a person-centered manner and integrate person-centered practices into their case manager supervision The Lead LTSS Case Manager will also assist with quality assurance activities and program development as well as serve as a PCT coach.

Requirements:

• Bachelor’s degree required; or Five years of relevant experience in the field of LTSS, which includes Developmental Disabilities; or Some combination of education and *relevant experience appropriate to the requirements of the position.
• Case management and/or advocacy strongly preferred.
• Knowledge of cognitive disabilities: cause, treatment, services for support, education, employment, recreation, therapy, and related medical terms/medications.
• Knowledge of forms and process for Long Term Medicaid services preferred.
• Supervisory experience preferred.

-1 full time position available at $28.91 – $31.80/hr. DOE/DOQ, current CO DSP minimum wage for overnights
