Lead Intake Case Manager

This position is responsible for the overall referral, intake, and assisting members with intellectual disabilities, or members who are elderly, blind and disabled and members with mental health illness. The Lead Intake Case Manager will assist with completing level of care (LOC) assessments. Responsibilities include supervision and training of the Intake/Waiting List Case Managers and activities associated with this position. Monitoring and tracking timelines to ensure the Colorado Intake Screen and LOC assessments are being completed within the state timelines. This position could have a temporary caseload and would provide case management services in a person-centered manner and integrate person-centered practices into the case manager supervision. This position participates in committees as assigned within the agency and as a liaison with ancillary agencies.
Requirements:
- Bachelor’s degree required or five years of relevant experience in the field of LTSS.
- Five years’ experience in Case Management and/or advocacy strongly preferred.
- Supervisory experience preferred.
- Driving is a requirement. Must have a valid driver’s license with an acceptable driving record.

– 1 full time position available at $28.91-$31.80/hr. DOE/DOQ.
