Case Management Aide – Children’s

The Children’s-CES/SLS Case Management Case Aide is responsible for a variety of duties including assisting Case Managers with enrollment and ongoing case management activities and administrative supports for the Children’s and SLS/CES programs. The Case Aide also assists the Foothills Gateway Family Support Council in coordinating council meetings, taking minutes, formatting reports, and assists in promoting and coordinating Family Support Council activities. The position may be responsible for completing primary case management activities such as monthly member contacts during periods of high member enrollment or case management turnover. Assists case managers, Members, and guardians with completing financial redetermination paperwork. May assist Case Managers with the completing the LOC Assessment if the Case Aide meets the education and experience requirements of the Case Manager position. The Children’s-SLS/CES Case Aide works in the Foothills Gateway building, in community settings, and has mobile work options. Spanish bilingual preferred with a $1.00 increase in pay rate.
Requirements:
- One year or more experience and/or training in office support preferred.
- Knowledge of HIPPA and confidentiality policies and rules preferred.
- Knowledge of Long-Term Service and Supports and Case Management preferred.
- Experience in document control, customer relations and contact with public.
