Case Manager

The Case Manager will be responsible for a variety of duties, which could include responding to contacts seeking information regarding services and supports for individuals with cognitive disabilities, assembling information about the individual for eligibility determination, providing Case Management services until the person can be admitted into services, being responsible for the planning, coordination, and monitoring of services and supports provided to individuals with cognitive disabilities and their families as appropriate. Spanish bilingual preferred with a $1.00 increase in pay rate. Driving is a requirement for this position. Bachelor’s degree required. Resume and cover letter required in addition to application.

$200 hiring bonus after 90 days of employment!

  • 2 full-time positions at 40 hrs. /wk., $21.14-$23.25/hr. DOE/DOQ
  • Remote/Mobile position–must be able to attend trainings and meetings at the main facility in Fort Collins, CO
