CASE MANAGEMENT CASE AIDE

The Case Management Case Aide will be responsible for a variety of duties, which includes responding to initial calls to Foothills Gateway regarding services and supports. Assessing and screening calls to find the best options for the member. Facilitating contact with FGI Intake Case Managers or LTSS Case Managers. Completion of Long-Term Care Assessments in times of high volume. Position will work in the Foothills Gateway building, in community settings, and have mobile work options. Spanish bilingual preferred with a $1.00 increase in pay rate.

 

Requirements:

  • Bachelor’s or associate degree helpful but not required. One year or more experience and/or training in office support preferred.
  • Experience in customer service.
  • Must complete and pass all FGI trainings required for this position.
  • Provide a courteous, professional contact for the site. Refer calls to appropriate agencies, if needed. Use discretion in screening all calls.
  • Must have good auditory skills in verbal cues/language to respond to interested applicants whether in person, by email or on the telephone.
  • Must have good verbal and written skills to communicate with potential members receiving services, families, other agencies staff, and public.
  • Complete Case Management Training for Contract Deliverables to assess for Long Term Care functional eligibility.
  • Monitor and follow up with members that were unable to complete referral within the first contact.
  • Facilitate contact with Lead Intake Case Manager or Lead LTSS Case Manager to assign a case manager for next steps in eligibility and enrollment process.
  • Driving is a required function of this job.
  • Knowledge of HIPPA and confidentiality policies and rules preferred.
  • Knowledge of Long-Term Service and Supports and Case Management preferred.
  • Experience in document control, customer relations and contact with public.
