Service and Support Program Highlights Foothills Gateway, Inc. (FGI) coordinates and provides services and supports for residents of Larimer County who are determined eligible for services through the cognitive disabilities system. Currently, more than 275 employees serve approximately 1,200 eligible individuals and their families. Funding for services and supports comes from county, state, federal and private funds. Assistance is given to individuals and their families either through providing services directly or by purchasing such services from other program approved service agencies (PASA). Services FGI provides directly are as follows: Resource Coordination/Case Management: This is the point of entry for individuals seeking services and supports. A skilled staff of Case Managers obtain information to document eligibility for services and then assist individuals and their families with developing a plan for the future and getting connected with services and supports. Case Managers actively coordinate and monitor the services being provided. Case management works with people receiving FGI services, people receiving services through other service agencies, and people waiting for services. Early Childhood Connections coordinates a variety of funding sources for services for children with disabilities (ages 0-3) and their families who meet the eligibility requirements for the program. There are several components of the program, and the families can access those that are needed as documented on their Individualized Family Support Plan. Family Support Services Program (FSSP) provides information, support, coordination and funding to families who have a member of any age with a cognitive disability living in the family home. Examples of services covered are respite, co-payments for medical insurance, professional services, recreation, adaptive equipment and home modifications. Supported Living Services (SLS): Supported Living Services are provided to individuals in order to assist them in maintaining or establishing independence. This program provides support to enable adult participants to make choices, receive needed services and live more independently in their own home or in the family home. Individuals are able to choose and customize their service/support options to meet their specific needs. The goal is to promote a sense of belonging and contribution to the community. Residential Services & HUD Program Administration: Committed to promoting a sense of community involvement and belonging for people with cognitive disabilities, residential services are developed to meet the needs of each individual in any of the settings we offer. Participants may live in apartments or houses and are supported by agency staff or host families. FGI also administers a Section 8 Subsidy (HUD) program which enables participants to afford community housing by subsidizing their rent with federal funds.
Day Programs Supported Employment Services: There are two successful service models offered through FGI’s employment program: individualized employment services and group employment services. Individualized employment services works with each individual to ensure success at a community job by assisting and consulting with both the business and individual to encourage a productive community employment experience. The group supported employment services offers a highly structured work environment for individuals with direct supervision by FGI staff. Examples of this include cleaning crews and business enclaves. Work Services: At Foothills Gateway, Inc., contract work procured from local businesses gives individuals with cognitive disabilities the opportunity to gain valuable work skills while earning wages. Facility-based opportunities for paid work include assembly, parts packaging, mailing services, janitorial, shipping and receiving, etc. Community Participation Services provides opportunities for individuals to participate in their communities through recreation/leisure programs, paid employment with on-going support, and volunteer positions. This allows individuals to become more comfortable in participating in community activities, as well as, allows the general community to witness the contributions of the individuals. Special Habilitation Services: For individuals who need more intensive support and supervision, FGI provides site-based services (activities of daily living, sensory and recreational activities) and community-based activities to enhance and improve their quality of life. Transportation services are provided for individuals between their home and program or work settings. Transportation is developed to best fit the needs and abilities of the individual. Whenever possible and feasible, existing community transportation options are used. Adult Care Services: This program provides families with temporary relief from ongoing care giving for their adult child (over age 18) with cognitive disabilities. Services are provided in a typical home-like setting and can be provided for up to eight (8) individuals during the daytime and three (3) individuals overnight. Participants in the Adult Care Services program engage in active habilitation and have opportunities to work on hygiene skills, educational activities, recreational activities, home maintenance activities, cooking activities, and additional activities to increase their independence. Adult Care Services reduces family stress and helps preserve the family unit, supports family stability, and prevents lengthy and costly out-of-home placements.
