Foothills Gateway is the Community Centered Board for Larimer County, one of 20
in the State of Colorado. As the Community Centered Board, Foothills Gateway
serves as the single entry point into services for individuals with cognitive or
developmental disabilities. If you would like to learn more about how to enroll
into services, contact our intake department at
970.226.2345. An intake case manager can help you with the application into
services, assist with the eligibility paperwork as well as provide additional
