Foothills Gateway Direct Services

Foothills Gateway provides direct services to clients in Larimer County. Here is a description of the service options we provide to clients.

  • Adult Care Services 
    This program provides short term relief from daily care giving responsibilities in a home-like setting for individuals age 18 and older. Adult Care Services (ACS) is open for respite services and care 24 hours a day, seven days a week (closed for some holidays). Families can schedule a short period of time within one day or up to 14 days. Day program is also available at ACS, Monday through Friday, 9am – 3pm for those eligible under the Elderly, Blind and Disabled Waiver. Activities are provided on site and in the community. Staff support and encourage learning and increasing living, socialization and communication skills. Individuals on the waiting list, in EBD, in Supported Living Services or private pay funds can receive this service.
  • Community Skill Development Program 
    This program, for individuals age 18 and older, focuses on a combination of recreational and vocational activities that challenge individuals and connect them to resources within the community. Individuals may participate in some paid employment in addition to volunteer opportunities and recreation opportunities. Staff provide support in skill training in activities of daily living, mobility training, communication, socialization, and recreation. Individuals can receive these services as part of their Supported Living Services plan or Comprehensive Services plan.
  • Residential Services 
    Committed to promoting a sense of community involvement and belonging for people with cognitive/developmental disabilities, residential services are developed to meet the needs of each individual in a variety of settings. Participants may live in apartments, companion settings, host homes, staffed homes or in group homes. This service is for individuals age 18 and older and receiving Comprehensive Services.
  • Special Habilitation Services 
    A program that focuses on increase self-sufficiency and/or basic skills to enable individuals to achieve an increase level of independence for individuals age 18 and older. Activities are highly therapeutic in nature and include sensory integration groups, a retirement group and an intensive supports group. Small groups travel into the community to access and participate in activities of their choice and interest. Individuals can receive these services as part of their Supported Living Services plan or Comprehensive Services plan.
  • Supported Employment Services (SE) 
    Foothills Gateway’s Supported Employment Services Program can provide assessments, job development, job placement, job coaching and follow up job support services. Supported Employment can be offered as individualized placements, enclaves or work crews. This program is for individuals age 18 and older. Individuals can receive these services as part of their Supported Living Services plan or Comprehensive Services plan.  View  the SE Video!
  • Work Services 
    This program provides a facility based opportunity for work for individuals age 18 and older. Contract work is procured from a variety of local businesses and may include electronic assembly, parts packaging, labeling, mailing services and product assembly. Individuals can receive these services as part of their Supported Living Services plan or Comprehensive Services plan.
  • Transportation 
    Services are provided for individuals age 18 and older between their home and program or work settings. Transportation is developed to best fit the needs and abilities of the individual. Whenever possible and feasible, existing community transportation options are used. Individuals can receive these services as part of their Supported Living Services plan or Comprehensive Services plan.
