Adult Services

Services for adults (clients over 18) are provided in primarily two models - Adult Supported Living and Adult Comprehensive Services.

  • Supported Living Services (SLS)  
    Adult Supported Living Services enable adult participants to make choices, receive needed services and live more independently in their own home or in the family home. Services available include: case management services, personal care services, supported employment services, respite and day program services. This program is for individuals age 18 and older.
    • Adult Care Services  

      This program provides short term relief from daily care giving responsibilities in a home-like setting for individuals age 18 and older. Adult Care Services (ACS) is open for respite services and care 24 hours a day, seven days a week (closed for some holidays). Families can schedule a short period of time within one day or up to 14 days. Day program is also available at ACS, Monday through Friday, 9am – 3pm for those eligible under the Elderly, Blind and Disabled Waiver. Activities are provided on site and in the community. Staff support and encourage learning and increasing living, socialization and communication skills. Individuals on the waiting list, in EBD, in Supported Living Services or private pay funds can receive this service.

  • Comprehensive Services  
    Adult Comprehensive Services provide 24-hour, seven day a week care for individuals age 18 and older. Service in an individual’s Service Plan include: case management services, residential care, employment or other day type services, and transportation. Individuals in Comprehensive Services must be eligible for Medicaid.
    • Residential Services  

      Residential - Comprehensive Service Program Approved Service Providers are responsible for developing living arrangements that fit the needs of each individual.

      • Apartment Setting - The individual lives in an apartment and the Program Approved Service Agency employs the staff who provide the support services. The staff will be accessible to the client on a daily basis for supervision and support.
      • Companion Setting  - A Companion/Live-In Aide is a full-time employee of a Program Approved Service Agency who lives with the individual and provides supervision in a home which is owned or leased by the client or the client’s family.
      • Host Home Setting - Host Homes (HH) are living environments which are leased or owned by the Host Home Provider (HHP). The term “Adult Foster Care” is often used to describe Host Homes Services. The Host Home Provider is under contract with a Program Approved Service Agency to provide Host Home services.
      • Residential Services/Staffed House - The Program Approved Service Agency owns or leases a house for 2 to 3 clients. Staff are employed by the Program Approved Service Agency and are available on a rotating basis to provide the supervision and support. Staff do not live in the home (although they may sleep in the home at night to provide supervision). The individual has his/her own bedroom.
      • Group Home - A Group Home is a home owned/leased by a Program Approved Service Agency and 4 to 8 clients can live in the home. The Program Approved Service Agency staffs the group home. Bedrooms can be shared. Group Homes are licensed by the Colorado Department of Health and Division for Developmental Disabilities (DDD).
    • Microboard  

      A Microboard is an agency created by family members or guardians, incorporated to do business in the State of Colorado, whose scope is solely to provide comprehensive services to one person with a developmental disability.

  • Community Activities/Day Services  
    Community Activities/Day Services provide training, support and supervision activities which maximize functional abilities and skills necessary to enable adults to access the community and/or provide the basis for building skills which will assist individuals to access the community.
    • Special Habilitation Services or Non-Integrated Activity  

      A program that focuses on increase self-sufficiency and/or basic skills to enable individuals to achieve an increase level of independence for individuals age 18 and older. Small groups travel into the community to access and participate in activities of their choice and interest. Individuals can receive these services as part of their Supported Living Services plan or Comprehensive Services plan.

    • Community Connection or Integrated Activity  

      This program, for individuals age 18 and older, focuses on a combination of recreational and vocational activities that challenge individuals and connect them to resources within the community. Individuals may participate in some paid employment in addition to volunteer opportunities and recreation opportunities. Staff provide support in skill training in activities of daily living, mobility training, communication, socialization, and recreation. Individuals can receive these services as part of their Supported Living Services plan or Comprehensive Services plan.

  • Vocational Employment Services  
    Vocational Employment Services include Supported Employment or site-based Work Services.  In each opportunity, individuals earn wages and valuable work skills.
    • Supported Employment Services  

      Supported Employment Services Program can provide assessments, job development, job placement, job coaching and follow up job support services. Supported Employment can be offered individualized placements, enclaves or work crews. This program is for individuals age 18 and older. Individuals can receive these services as part of their Supported Living Services plan or Comprehensive Services plan.

    • Work Services or Non-Integrated Work  

      This program provides a facility based opportunity for work for individuals age 18 and older. Contract work is procured from a variety of local businesses and may include electronic assembly, parts packaging, labeling, mailing services and product assembly. Individuals can receive these services as part of their Supported Living Services plan or Comprehensive Services plan.
