RFP - $15,000 for Demonstration Grant
8/25/2010 - Foothills Gateway, Inc. is pleased to announce that we are
interested in working with other organizations/groups in the Larimer County,
Colorado area on a demonstration project on the value of on-going supports, post
placement, in improving job retention in meaningful work settings, and our
ability to award a $15,000, 9 month grant to plan and implement a project in
this area. The target population for this grant would be individulas with
developmental disabilities who are eligible for services as defined in C.R.
27-10.5-102(11)(a); and who are transitioning from school to work, or who have
transitioned. Grant deadline is 4:00 p.m. on September 30th, 2010.

Foothills Gateway prepares for H1N1
Update 11/16/09 - The 2009 H1N1
vaccine clinic will be held at Foothills Gateway on December 4, 2009 from
9:30am to 2 pm. You may access the clinic by coming to the south west door.
Please follow the signs on Foothills Gateway property. The vaccine will be made
available to all Adult clients served by Foothills Gateway and their direct
residential providers. Please bring a copy of your Medicare or Medicaid card,
guardianship information and any allergy information. If you have questions
about the 2009 H1N1 vaccine clinic, please call 266-5450, leave a message and we
will return your call as soon as possible. Thank you.
10/27/09 - The Larimer County
Health Department is working with Foothills Gateway to make the 2009 H1N1
influenza vaccine more easily available to our adult clients. The vaccine will
be available to adult clients 18 years or older (who have graduated from school)
and who receive Comprehensive Services, Supported Living Services, or who are on
the Waiting List. There will be no cost to clients for this vaccine.
These clinics will be for adult clients only; family members and clients
younger than 18 years old should see their personal physician to determine the
need for vaccination.

4/30/09 - Foothills Gateway began
following the guidance of the Larimer County Department of Health and
Environment regarding the H1N1 (Swine Flu) outbreak, and highly recommends
prevention measures for clients, families and staff.
Foothills Gateway continues to plan for and evaluate the situation daily, and
continues to participate in the county planning effort underway for three years
with the County Pandemic Influenza Planning Partnership.
Some resources clients and families can consult for
updated information regarding H1N1 include the Centers for Disease Control and
Prevention as well as the
Colorado Department of
Public Health and Environment and
Larimer County Flu Information. We have also attached a
checklist for individuals and families to use as a reference tool if you
should wish.
