Family Caregiver model – November 2010 – CDHS/DDD is implementing the Family Caregiver model effective November 1, 2010.  Below you will find the initial guidelines for this implementation.  DDD will be continuing to add to these guidelines, based on input from the Family Caregiver work group, individuals in service, families, advocacy organizations, providers and case managers until DDD and stakeholders are satisfied with a final product. Should you have any questions about the Family Caregiver model, please contact your case manager.
