FULL TIME DIRECT SUPPORT PROVIDER II – CES/ACS:
Full time position working with both the Adult Care Services Program and the Children Extensive Services Program. Shifts available include day, evening, weekends and overnights. Experience is preferred. We are looking for staff who are committed to developing positive working relationships with children and their families and staff who are creative in planning and implementing a variety of activities. Staff will need to be able to learn routines and individuals supported in these programs in a fairly quick manner and demonstrate a great sense of versatility in their availability and attitude. CES DIRECT SUPPORT PROVIDER II
The person in this position works directly with children who have intellectual/developmental disabilities and are enrolled in the CES program. This person provides direct services in areas such as communication, daily living skills, education, exercise, recreation, and health hygiene as outlined in the child’s Service Plan and Prior Authorization Record; and, develops positive/proactive behaviors with children. Services would take place in the child’s home and/or in community settings. Daytime availability needed. Driving is a requirement for this position.
- 1 part-time position at 15-20 hrs./ week starting at $12.07/hr.

LIVE-IN DIRECT SUPPORT PROVIDER:
This is a unique position. The staff person in this position lives with the person receiving services in the person’s home. The DSP-Live In provides support to the person receiving services as stipulated by the Service Plan (SP) and the agency agreement. Compensation is based on the current minimum wage for the specific hours, services and supports that are provided. Sleep time is unpaid. Please contact Judy Tomcak for additional information about the pay. Additionally, the person in this position will receive a monthly food stipend that covers the cost of the DSP-Live In’s food. The DSP-Live In does not have to pay rent or utilities. The job duties include direct services in areas such as communication, daily living skills, education, exercise, recreation, and health and hygiene as outlined in the individual’s SP. Support will also involve developing positive/proactive behaviors in all settings and in the community. Driving is a requirement for this position.
- 1 F/T position starting at minimum wage/awake hours

DIRECT SUPPORT PROVIDER II - RESIDENTIAL:
The person in this position works directly with individuals who have intellectual/developmental disabilities. Duties include direct services in areas such as communication, daily living skills, education, exercise, recreation, personal hygiene and support to develop positive/proactive behaviors in all settings. Various scheduling options available. Driving is a requirement for this position.
- Part-time and on-call staffing starting at $11.11/hr.
- Can earn $12.07/hr. by working in enhanced homes with individuals with challenging behaviors.
- Some experience is preferred. Must be flexible and dependable.

ACTIVITY COORDINATOR I:
The person in this position works directly with individuals who have intellectual/developmental disabilities. The person in this position facilitates communication skills training, domestic skills training, educational/vocational skills training, exercise/health/hygiene skills training, and develops positive/proactive behaviors for individuals in group or individualized settings. The person in this position will perform his/her duties at Foothills Gateway and at various community sites. Driving is a requirement for this position.
- 2 full time position at 35 hrs./week in Community Skills Development starting at $10.59/hr.
- Includes 23 paid holidays per year in addition to paid time off and benefits!

DRIVER/VEHICLE MAINTENANCE TECH ASSISTANT:
The person in this position works directly with individuals who have intellectual/developmental disabilities. Responsibilities include but are not limited to assisting the Vehicle Maintenance Technician with monthly inspections of agency vehicles and driving passengers to and from places of employment, their residence and other locations. Driving is a requirement for this position and all duties are performed within the Foothills Gateway service area.
- 3 part-time positions up to 29.75 hrs./week – M-F
- The three positions that are available are from 6:30am-noon, 11am-4:30pm, and 1:30pm-7pm. Hours may adjust depending on the transportation needs for a specific day.

SUPPORTED EMPLOYMENT GROUP TRAINER:
Directs, trains, and works alongside individuals with cognitive disabilities in performing work assignments of mobile cleaning crews. The Group Trainer should be familiar with the training needs of individuals with disabilities and modify work assignments as needed to ensure the satisfactory completion of cleaning responsibilities. Responsible for assuring the quality of work completed at each residence or business site. Responsible for training individuals with I/DD. Also responsible for cleaning/janitorial duties. Facilitates the inclusion of individuals with disabilities into community settings. Driving is a requirement for this position.
- 1 full-time position at 40 hrs./week starting at $11.09/hr.

SUPPORTED EMPLOYMENT GROUP TRAINER-FEDERAL BUILDING:

The Group Trainer is responsible for assuring the quality of work completed at job site. Position is located at the U.S. Federal Building/Post Office in downtown Fort Collins. Hours are from1:00 p.m. - 6:00 p.m. Work to be performed on-site at the Fort Collins Federal Building at 301 S. Howes St. Must pass Federal background check. Driving is a requirement for this position.
- 1 part-time position starting at $11.15/hr. working 25 hrs./wk. from 1pm-6pm
