DIRECT SUPPORT PROVIDER II - RESIDENTIAL:
The person in this position works directly with individuals who have intellectual/developmental disabilities.
This person provides direct services in areas such as communication, daily living skills, education, exercise,
recreation, and health and hygiene and supports individuals to develop positive/proactive behaviors in all settings.
Flexible hours. Will work with your schedule. For the Residential Direct Support Provider position, you must read
and initial the Job Description and Physical Demands Evaluation. These may only be obtained at Foothills Gateway
and must be submitted with your application packet.
Now hiring for part time and on-call staffing $11.11/hr.
Can earn $12.07/hr. by working in enhanced homes with individuals with challenging
behaviors. Some experience is preferred. Must be flexible and dependable.

EARLY INTERVENTION SERVICE COORDINATOR:
The Early Intervention/Part C Service Coordinator is responsible for planning,
coordinating, and monitoring of services and supports provided to families of infants and
toddlers eligible for Federally funded Part C services and State funded Early Intervention
services. The EI Service Coordinator shall have an assigned caseload of individuals for
whom he/she shall be responsible to advocate; schedule and facilitate initial Routines
Based Interviews (RBI), Child Find assessments, Interdisciplinary Team (IDT) meetings to
develop and, as needed modify an Individualized Family Service Plan (IFSP); monitor the
provision of services, supports and recommendations identified in the IFSP, assist in
obtaining identified services and supports; maintain a master record and all necessary
documentation; serve as the primary contact for obtaining and disseminating information;
and facilitate and mediate conflict resolution. Inherent in all of these functions shall
be activities supporting families rights and procedural safeguards, support, empowerment,
and advocacy. Bachelor's degree in social work, human services, or related field required.
